4K a year 4 days ago 4 days ago 4 days ago As a Lead Quality Improvement Support you will be responsible for providing direct support to HealthLinc's Quality Improvement department to include assisting with all activities that support the Center's key clinical quality improvement initiatives.
You will collect and analyze quality data in preparation of quality reports and assist in tracking clinical data.
This position will work with the clinical data and clinic staff and will report to the Quality Manager - Improvement.
JOB
Responsibilities:
In collaboration with the Director of Quality Improvement, coordinates tracking and reporting of clinical outcomes and follow-up.
Researches topics for new development and quality improvement and collaborate with the Director of Quality Improvement for approval and implementation.
Serves as mentor for Quality Improvement staff.
Department liaison to build relationships with clinics for best outcomes.
Assists and trains in running reports in Azara to support department needs.
Coordinates and oversees preventative cancer screening grants.
Oversee that Quality Improvement Support monthly tracking and tasks are on track to meet annual goals and appropriate resolutions.
This includes meeting with staff regarding their tracking and progress toward goals in coordination with Quality Manager.
Helps perform audits on patient medical records at all health centers.
Assists in scheduling audits performed by outside agencies, as needed.
Helps target low-performing indicators with improvement strategies and track changes over time to ensure strategies have been effective.
Assists in contacting patients in need of identified health services and assist in scheduling their appointments.
Tracks outcomes and provide monthly reports.
Attendance is an essential function of the job.
Attends and participate in assigned committees.
Attends mandatory training as required.

REQUIRED
Qualifications:
Education/Training Bachelor's degree in Public Health, Pre-Med, or applicable major preferred Proven experience with Excel spreadsheets, graphs, and flowcharts Skills/Job Requirement Strong organizational and time management skills Proven ability to work well in a team environment Excellent written and verbal communication skills Ability to remain flexible and adaptable Ability to follow HealthLinc policies and procedure Technology Skills Operate a multi-line phone system and other office equipment including printers, fax machines, etc.
Basic software skills (Microsoft Office, EHR, online sources, etc.

Estimated Salary: $20 to $28 per hour based on qualifications.
